1  The Phillips 66-Conoco-76 Premium Card Membership Kit will include details on all program features.  The Premium Card Program is

2  You must make all reservations through the Phillips 66-Conoco-76 Travel Center, at least 5 days in advance for airline and 3 days in
    advance for car and hotel, to receive your “Cash-Back.”  Cash-Back is based on the base rate and doesn’t include taxes, surcharges or
    processing fees.  There is a processing fee per ticket for all airline tickets booked.  Confirmations will be sent directly to you—you cannot
    purchase tickets at the airport.

3  Reservations for conventions, conference accommodations, travel industry representatives rates, charter travel, consolidators,
    compassion fares, government rates, bereavement fares or frequent flyer redemption do not qualify for Cash-Back rebates.

4  You may earn up to a maximum of $50 in rebates each month.  Rebates are not earned on purchases made at locations that sell
    non-Phillips 66, Conoco or 76 branded gasoline.  Rebates are automatically credited on your statement against future Phillips 66,
    Conoco and 76 purchases made with your card.  See the Phillips 66-Conoco-76 MasterCard Rebate Program Information for more details.

5  The balance transfer APR is 2.99% for 6 months from the date of the first balance transfer. After the promotional period ends, your standard
    APR will be applied to any unpaid balances. The standard variable purchase APR is 11.24% for Platinum and 15.24%
    for Classic. The standard variable APR for cash advances is 19.99%. However, all your APRs may automatically increase to the 27.24%
    variable default rate if you default under any Card Agreement that you have with us. We may also increase the promotional rate if you fail
    to make payments to another creditor when due. Minimum finance charge $0.50. Fee for Foreign Purchases-3% of the U.S. dollar amount of     each purchase made outside the U.S., whether made in U.S. dollars or in a foreign currency. Transaction fee for cash advances: 3% of the     amount of each cash advance, minimum $5.00.
    Transaction fee for balance transfers: 3% of the amount of each balance transfer, minimum $5.00.
