2 You must make all reservations through the Phillips 66-Conoco-76 Travel Center, at least 5 days in advance for airline and 3 days in
advance for car and hotel, to receive your “Cash-Back.” Cash-Back is based on the base rate and doesn’t include taxes, surcharges
or processing fees. There is a processing fee per ticket for all airline tickets booked. Confirmations will be sent directly to you—you
cannot purchase tickets at the airport.
3 Reservations for conventions, conference accommodations, travel industry representatives rates, charter travel, consolidators,
compassion fares, government rates, bereavement fares or frequent flyer redemption do not qualify for Cash-Back rebates.
4 The $100,000 Automatic Flight Insurance, Policy No. 6477-44-64, and the $25,000 Common Carrier Insurance, Policy
No. 6404-55-20 are underwritten by Federal Insurance Company, a member insurer of Chubb Group of Insurance Companies,
